I, the undersigned, understand that Bellabing, Inc., Art From The Heart, and the Art From The Heart staff are not responsible in any way whatsoever for any accidents or injuries that may occur while attending classes, or on studio property at any time. I also understand that this disclaimer applies to any out of class performances being held. As such, I understand that no monetary compensations can be sought from either Bellabing, Inc., Art From The Heart, or the Art From The Heart staff related to previously mentioned accidents or injuries. By signing, I also accept the terms and conditions of payment, as stated being due by the 1st class of each month, with a $15 late fee per student being incurred for late payments, and will abide by all established rules / procedures. I agree that I have read, understand and agree to all facility rules and requirements, tuitions and payments, policy and procedures. I also understand that any non-compliance with the above could result in my child’s removal from ART from the HEART and the possibility of 3rd party assistance in collecting any unpaid monies due AFTH.
