The following are some of the most commonly requested documents at AFTH.

We thought we would include a place on the website where you can download what you need - when you want it.

Refer A Friend Pass (1 free class)

Credit Card Auth Form (Single use)

Credit Card Auth Form (auto-pay/multi-use)

Uniform Exchange Form

Use this form to give to a friend so that they can come and try an AFTH class for free!

If they end up registering and joining AFTH, you will receive a $15 credit on your AFTH account!

Use the Single Use Credit Card Form to make a one time payment on your AFTH Account

Use the Multi-use Credit Card form if you would like to authorize auto payments for monthly tuition, costume deposits, etc.

Use this form if you need to exchange any retail orders such as shoes, uniforms, etc.

Please keep in mind, an returns must be in an unused condition with no wear.  AFTH office will have final determination if retail is suitable for return.
